I confirm my dog has been vaccinated, has an up to date flea treatment, and I have outlined any health conditions and/or adverse behaviour issues on this form or the groom form. I understand that sometimes situations occur that are out of the control of Pooch in Paradise. While all care is taken of my dog/any unfortunate incident or injury that requires a vet visit for my dog will not be paid by Pooch in Paradise but by me. I understand that pet insurance is a wise idea before booking.
